If you need a Certification of Foreign Retirement form, please visit the City Attorney’s office in City Hall, 800 Dunlop Road. Please note this form must be obtained from the City, certified by the City Clerk, and notarized. We ask that you please call the City Attorney’s office at 239-472-4359 prior to your visit.
